{"id":163433,"date":"2024-06-26T14:22:12","date_gmt":"2024-06-26T20:22:12","guid":{"rendered":"https:\/\/tecscience.tec.mx\/en\/?p=163433"},"modified":"2024-07-11T12:16:00","modified_gmt":"2024-07-11T18:16:00","slug":"improving-medical-care-towards-the-lgbtiqa-community","status":"publish","type":"post","link":"https:\/\/tecscience.tec.mx\/en\/human-social\/improving-medical-care-towards-the-lgbtiqa-community\/","title":{"rendered":"How to Improve Medical Care for the LGBTIQA+ Community"},"content":{"rendered":"\n<p>In December 2022, <strong>Andr\u00e9s<\/strong> \u2013a transgender man who has asked <strong><em>TecScience<\/em><\/strong> to protect his real name\u2013 attended clinical laboratories in Mexico City to have an ultrasound to ensure that there were no abnormalities in his mammary glands before undergoing subcutaneous mastectomy as part of his gender reaffirmation process.<\/p>\n\n\n\n<p>\u201cNormally, it would have taken 25 minutes, but it took an hour and a half due to a lack of staff training. They took me to a room and asked me more questions,\u201d says Andr\u00e9s. \u201cThey spoke to me using she\/her pronouns and treated me in a derogatory manner.\u201d<\/p>\n\n\n\n<p>Before leaving, his mother, who was accompanying him, filed a complaint with the place&#8217;s supervisors for discrimination.<\/p>\n\n\n\n<p>\u201cThey told me, in a loud voice, that it was my fault because I hadn&#8217;t shaved before my appointment. It was embarrassing; all the people who were waiting for their turn found out.\u201d<\/p>\n\n\n\n<p>Being treated like that left him feeling uneasy. \u201cSince then, if I need to go to a doctor, I make sure through people I know that they are allies of trans and queer people.\u201d<\/p>\n\n\n\n<p>His case is just one of many examples of how medical and health personnel sometimes discriminate against members of the LGBTIQA+ community.<\/p>\n\n\n\n<figure class=\"wp-block-embed is-type-wp-embed is-provider-tecscience wp-block-embed-tecscience\"><div class=\"wp-block-embed__wrapper\">\n<blockquote class=\"wp-embedded-content\" data-secret=\"2u2yOUitEr\"><a href=\"https:\/\/tecscience.tec.mx\/en\/human-social\/inclusion-drives-scientific-innovation\/\">How Inclusion Drives Scientific Innovation<\/a><\/blockquote><iframe class=\"wp-embedded-content\" sandbox=\"allow-scripts\" security=\"restricted\" style=\"position: absolute; clip: rect(1px, 1px, 1px, 1px);\" title=\"&#8220;How Inclusion Drives Scientific Innovation&#8221; &#8212; TecScience\" src=\"https:\/\/tecscience.tec.mx\/en\/human-social\/inclusion-drives-scientific-innovation\/embed\/#?secret=K5e8lqZYa7#?secret=2u2yOUitEr\" data-secret=\"2u2yOUitEr\" width=\"600\" height=\"338\" frameborder=\"0\" marginwidth=\"0\" marginheight=\"0\" scrolling=\"no\"><\/iframe>\n<\/div><\/figure>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Medical Care for the LGBTIQA+ Community<\/strong><\/h2>\n\n\n\n<p>Five years before Andr\u00e9s&#8217; case, the Ministry of Health in Mexico published the first <a href=\"https:\/\/www.gob.mx\/insalud\/documentos\/protocolo-comunidad-lgbtti\" target=\"_blank\" rel=\"noreferrer noopener\">Protocol for Access without Discrimination to the Provision of Health Care Services for LGBTTI people<\/a>.<\/p>\n\n\n\n<p>There are many other guidelines from health institutions focused on providing quality care to members of this community, but discrimination is still common.<\/p>\n\n\n\n<p>\u201cI think it starts with the pronouns; we should ask them [at the beginning of the consultation], but it is something we don&#8217;t do,\u201d says<strong> Amairany Vega<\/strong>, a surgeon at the <a href=\"\/en\/tag\/school-of-medicine-and-health-sciences\/\" target=\"_blank\" rel=\"noreferrer noopener\">School of Medicine and Health Sciences<\/a> at Tec de Monterrey.<\/p>\n\n\n\n<p>Vega, a bisexual woman, led <a href=\"https:\/\/www.igi-global.com\/chapter\/a-generation-of-healthcare-professionals-sensitive-to-the-needs-of-the-lgbtq-population\/341446\" target=\"_blank\" rel=\"noreferrer noopener\">a chapter<\/a> in the book <a href=\"https:\/\/books.google.com.mx\/books?id=kAj6EAAAQBAJ&amp;pg=PA164&amp;lpg=PA164&amp;dq=Shake+it+off:+Emotional+keys+for+a+successful+performance+in+the+clinical+environment&amp;source#v=onepage&amp;q=Shake%20it%20off%3A%20Emotional%20keys%20for%20a%20successful%20performance%20in%20the%20clinical%20environment&amp;f=false\" target=\"_blank\" rel=\"noreferrer noopener\"><em>The Lifelong Learning of Health Professionals: Continuing Education and Professional Developmen<\/em>t<\/a>, on how to raise awareness about community care in future generations.<\/p>\n\n\n\n<p>She and her colleagues explain that discrimination against this population by health personnel can take many forms.<\/p>\n\n\n\n<p>Among them are not using the appropriate pronouns, assuming the sexual orientation of patients due to their behavior, making judgments regarding their sexual activity, and omitting questions due to ignorance of other sexual practices that are different from heterosexual vaginal intercourse.<\/p>\n\n\n\n<p>This can result in misdiagnoses, refusal of certain exams or tests, or prejudiced attribution of an illness.<\/p>\n\n\n\n<p>\u201cIt is still assumed that some diseases, such as HIV, are transmitted more between homosexual men, but there is no scientific support for this,\u201d explains Vega.<\/p>\n\n\n\n<p>One of the most severe consequences of this discrimination is that people who belong to this community prefer not to go to the doctor.<\/p>\n\n\n\n<p>\u201cMy experience is similar to that of many trans people who try to minimize their contact with any type of doctor,\u201d says Andr\u00e9s.<\/p>\n\n\n\n<p>In Mexico, compared to the rest of the population, 5.9% more LGBTIQA+ people seek medical care in the private sector, and 2.1% do not seek it in any setting, according to the <a href=\"https:\/\/www.inegi.org.mx\/contenidos\/programas\/endiseg\/2021\/doc\/endiseg_2021_resultados.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">National Survey on Sexual and Gender Diversity <\/a>(ENDISEG).<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>The First Step Towards Non-Discrimination is in the Academy<\/strong><\/h2>\n\n\n\n<p>According to Vega, it all starts in school, when medical students learn about diseases&#8217; theoretical and practical aspects.<\/p>\n\n\n\n<p>\u201cI think that, even though there is already<a href=\"https:\/\/www.researchgate.net\/publication\/319906118_A_first_step_in_addressing_medical_education_Curriculum_gaps_in_lesbian-_gay-_bisexual-_and_transgender-related_content_The_University_of_Louisville_Lesbian_Gay_Bisexual_and_Transgender_Health_Certifi\" target=\"_blank\" rel=\"noreferrer noopener\"> research<\/a> on the matter, there is still a gap between what is said and what is done,\u201d says <strong>Pilar Gonz\u00e1lez<\/strong>, Leader of Preventive and Training Strategies at the Center for the Recognition of Human Dignity (CRDH) of <a href=\"\/en\/tag\/tecsalud\/\" target=\"_blank\" rel=\"noreferrer noopener\">TecSalud<\/a>.<\/p>\n\n\n\n<p>Therefore, teaching the medical community about how to care for LGBTQI+ people empathically continues to depend on teachers or students who are sensitive to the issue.<\/p>\n\n\n\n<p>\u201cThe medical discipline has very patriarchal, simplistic, and binaristic features. It was established as a rigorous science, but is not self-critical; it remains blind and stubborn to admit the relevance of social sciences in its study. There is also a remnant of some religious values \u200b\u200bthat are too focused on sexuality,\u201d Gonz\u00e1lez points out.<\/p>\n\n\n\n<p>Vega adds that non-heterosexual practices are minimized \u201cas if they were an exception, but they are not.\u201d<\/p>\n\n\n\n<p>In Mexico, 5.1% of the population aged 15 years or older identifies as part of the LGBTIQA+ community, according to ENDISEG.<\/p>\n\n\n\n<p>Therefore, updating study plans and preparing those who teach is essential. \u201cCurrently, the faculty is not qualified to teach courses on these topics. That effort is also needed,\u201d says Vega.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>How to Eradicate Discrimination Against the LGBTIQA+ Community in the Medical Environment<\/strong><\/h2>\n\n\n\n<p>To eradicate poor care and discrimination towards the LGBTQI+ community by health personnel, a coordinated effort is needed from various spheres of society, but, above all, from decision makers.<\/p>\n\n\n\n<p>\u201cIt is urgent to raise awareness among the people making the decisions, both in schools and in medical practice,\u201d says Gonz\u00e1lez.<\/p>\n\n\n\n<p>Representing LGBTIQA+ people within hospitals, medical schools, and practices is equally important.<\/p>\n\n\n\n<p>\u201cCurrently, [hospitals and medical scenarios] are not such safe spaces for them to demonstrate their diversity. Mechanisms need to be put in place to protect those who are vocal about their sexual or gender identity,\u201d she adds.<\/p>\n\n\n\n<p>Efforts must also be focused on encouraging scientific research with a gender perspective. \u201cThe stigmas about gender-based dissidence are powerful; they must be broken with evidence,\u201d says Gonz\u00e1lez.<\/p>\n\n\n\n<p>It is also key to invite people from the medical community to seek to improve their relationships with their patients and their care for this community.<\/p>\n\n\n\n<p>\u201cPeople come for consultation when they are vulnerable, sick or in pain,\u201d says Gonz\u00e1lez. \u201cWe should be particularly empathetic toward everyone.\u201d<\/p>\n\n\n\n<p>Although medical professionals often have complicated schedules, it is time to prioritize the care of the most vulnerable communities.<\/p>\n\n\n\n<p>\u201cI know that sometimes they don&#8217;t have the time to do these trainings. I don&#8217;t think they do it maliciously, but they can change how you go to the doctor and whether you go or not in the first place,\u201d says Andr\u00e9s.<\/p>\n\n\n\n<p>Although the panorama sounds complex, according to Gonz\u00e1lez and Vega, it is encouraging that new generations seek to break with the care paradigms that have prevailed until now.<\/p>\n\n\n\n<p>\u201cAs health professionals, we are important in treating diseases, but also in accompanying the people we care for,\u201d says Vega.<\/p>\n\n\n\n<p><strong><em>Were you interested in this story?
