But a <a href=\"https:\/\/www.frontiersin.org\/journals\/pediatrics\/articles\/10.3389\/fped.2025.1597309\/full\" target=\"_blank\" rel=\"noreferrer noopener\">new study led by Mexican researchers is challenging that approach<\/a>, calling on the medical community to rethink how we protect children\u2019s health.<\/p>\n\n\n\n<p>The research was inspired by a recent report from <strong>The Lancet &amp; Endocrinology Commission<\/strong>, where an international group of scientists <a href=\"https:\/\/www.thelancet.com\/journals\/landia\/article\/PIIS2213-85872400316-4\/abstract\" target=\"_blank\" rel=\"noreferrer noopener\">proposed modernizing how we classify weight-related metabolic disorders<\/a>.<\/p>\n\n\n\n<p>Instead of labeling people as \u201cnormal weight,\u201d \u201coverweight,\u201d or \u201cobese,\u201d they suggest a new system: metabolically healthy, preclinical obesity, and clinical obesity.<\/p>\n\n\n\n<p>The authors also propose going beyond <a href=\"\/en\/health\/definition-of-clinical-obesity\/\" target=\"_blank\" rel=\"noreferrer noopener\">BMI<\/a> by including measurements of muscle mass and metabolic markers\u2014such as blood glucose levels\u2014to determine better whether a person, especially a child, has a metabolic condition.<\/p>\n\n\n\n<figure class=\"wp-block-image aligncenter size-large\"><picture>\r\n                <source srcset=\"https:\/\/tecscience.tec.mx\/en\/wp-content\/uploads\/sites\/9\/2025\/08\/body-mass-index-819x1024.webp\" type=\"image\/webp\">\r\n                <img fetchpriority=\"high\" decoding=\"async\" width=\"819\" height=\"1024\" src=\"https:\/\/tecscience.tec.mx\/en\/wp-content\/uploads\/sites\/9\/2025\/08\/body-mass-index-819x1024.jpg\" alt=\"\" class=\"wp-image-170773\" srcset=\"https:\/\/tecscience.tec.mx\/en\/wp-content\/uploads\/sites\/9\/2025\/08\/body-mass-index-819x1024.jpg 819w, https:\/\/tecscience.tec.mx\/en\/wp-content\/uploads\/sites\/9\/2025\/08\/body-mass-index-240x300.jpg 240w, https:\/\/tecscience.tec.mx\/en\/wp-content\/uploads\/sites\/9\/2025\/08\/body-mass-index-768x960.jpg 768w, https:\/\/tecscience.tec.mx\/en\/wp-content\/uploads\/sites\/9\/2025\/08\/body-mass-index.jpg 1080w\" sizes=\"(max-width: 819px) 100vw, 819px\" \/>\r\n            <\/picture><\/figure>\n\n\n\n<h2 class=\"wp-block-heading\">BMI is Simple, But Often Misleading<br><\/h2>\n\n\n\n<p><strong>BMI is calculated by dividing a person\u2019s weight in kilograms by their height in meters squared<\/strong>. For adults, that number directly places them in a category. For children, it\u2019s compared to standardized growth charts based on age and sex.<\/p>\n\n\n\n<p>But here\u2019s the problem: BMI can\u2019t distinguish fat from muscle.<\/p>\n\n\n\n<p>\u201cSome people score high on BMI, but they\u2019re not actually obese,\u201d explains <a href=\"https:\/\/research.tec.mx\/vivo-tec\/display\/PID_479655\" target=\"_blank\" rel=\"noreferrer noopener\">Cipatli Ayuzo<\/a>, a pediatrician and researcher at <a href=\"\/en\/tag\/tecsalud\/\" target=\"_blank\" rel=\"noreferrer noopener\">TecSalud<\/a> and Hospital Zambrano Helli\u00f3n. \u201cThink about athletes like Canelo \u00c1lvarez or The Rock\u2014they have a lot of muscle mass.\u201d<\/p>\n\n\n\n<p>The new study, which Ayuzo co-authored, proposes a more complete method for evaluating metabolic health in children.<\/p>\n\n\n\n<figure class=\"wp-block-embed is-type-video is-provider-youtube wp-block-embed-youtube wp-embed-aspect-16-9 wp-has-aspect-ratio\"><div class=\"wp-block-embed__wrapper\">\n<iframe title=\"Childhood Obesity: what solutions does the science of the future promise?\" width=\"800\" height=\"450\" src=\"https:\/\/www.youtube.com\/embed\/YU49W8Y4Ju4?feature=oembed\" frameborder=\"0\" allow=\"accelerometer; autoplay; clipboard-write; encrypted-media; gyroscope; picture-in-picture; web-share\" referrerpolicy=\"strict-origin-when-cross-origin\" allowfullscreen><\/iframe>\n<\/div><\/figure>\n\n\n\n<h2 class=\"wp-block-heading\">Not Every Child with a High BMI is Unhealthy<\/h2>\n\n\n\n<p>The team recruited 111 physically active children, ages 6 to 11, who played in a soccer league in Monterrey, Mexico.<\/p>\n\n\n\n<p>Each child underwent several tests, including weight, height, bone mass, body fat percentage, and muscle mass. Researchers also collected blood samples to measure glucose and cholesterol levels.<\/p>\n\n\n\n<p>Using this broader set of data, the team applied the 2025 Obesity Classification Framework (2025-OCF)\u2014a new diagnostic tool inspired by The Lancet report\u2014to offer a more accurate view of each child\u2019s health.<\/p>\n\n\n\n<p><strong>The contrast was striking:<\/strong><\/p>\n\n\n\n<p>Using BMI alone, 48 children were classified as normal weight, 40 as overweight, and 23 as obese.<\/p>\n\n\n\n<p>Using the new classification, 56 were categorized as normal weight, 51 as having preclinical obesity, and only 4 as having clinical obesity.<\/p>\n\n\n\n<p>\u201cThere were kids with a lot of muscle who had no metabolic issues,\u201d says Ayuzo, \u201cand others who looked healthy but had concerning metabolic markers.\u201d<\/p>\n\n\n\n<p>Some of the children previously labeled as overweight or obese simply had more muscle or heavier bones. Meanwhile, some thinner kids showed early signs of metabolic imbalance.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">What this Means for Parents, Pediatricians, and Policymakers<br><\/h2>\n\n\n\n<p>Looking ahead, the researchers recommend supplementing BMI with other indicators\u2014such as waist circumference or bioimpedance scales that estimate body fat percentage. This is especially important for children who play sports or have larger body frames.<\/p>\n\n\n\n<p>They also call for a less simplistic view of weight:<\/p>\n\n\n\n<p>\u201cNot every child with a high BMI is at risk, and not every child with a \u2018normal\u2019 BMI is healthy.\u201d<\/p>\n\n\n\n<p>These findings serve as a reminder that children\u2019s health can\u2019t be measured with a one-size-fits-all approach. By analyzing both the body and metabolism more thoroughly, we can avoid misdiagnoses, unnecessary interventions\u2014and perhaps most importantly, detect risks early and without bias.<\/p>\n\n\n\n<p>\u201cIf a child goes to the doctor and is told they\u2019re fat or obese, that label can stick with them for life,\u201d Ayuzo warns. \u201cAnd maybe all they had was extra muscle mass.\u201d<\/p>\n\n\n\n<p>In a world where childhood obesity rates are rising, having fairer, more precise tools isn\u2019t just a technical upgrade\u2014it\u2019s a better way to care for our kids.<\/p>\n\n\n\n<p>\u201cThis study has the scientific foundation to say: \u2018I measured every aspect, and I can tell you this is a healthy child\u2014just with a bigger body,\u2019\u201d says Ayuzo.<\/p>\n\n\n\n<p><em><strong>Interested in republishing this story?
