{"id":172506,"date":"2026-02-18T12:44:55","date_gmt":"2026-02-18T18:44:55","guid":{"rendered":"https:\/\/tecscience.tec.mx\/en\/?p=172506"},"modified":"2026-02-25T17:53:43","modified_gmt":"2026-02-25T23:53:43","slug":"treatment-for-amyotrophic-lateral-sclerosis","status":"publish","type":"post","link":"https:\/\/tecscience.tec.mx\/en\/health\/treatment-for-amyotrophic-lateral-sclerosis\/","title":{"rendered":"A Mexican Stem Cell Protocol to Treat Amyotrophic Lateral Sclerosis (ALS)"},"content":{"rendered":"\n<p>Amyotrophic lateral sclerosis (ALS) is one of the most devastating neurodegenerative diseases, with a <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC12012833\/\" target=\"_blank\" rel=\"noreferrer noopener\">life expectancy of two to five years<\/a> after the onset of the first symptoms. In Mexico, a group of researchers has shown that an <a href=\"https:\/\/www.researchgate.net\/profile\/Jorge-Moreno-Cuevas\/publication\/299377825_Long-term_survival_in_amyotrophic_lateral_sclerosis_after_stem_cell_transplantation_into_the_frontal_motor_cortex\/links\/577009fb08ae0b3a3b7b8f46\/Long-term-survival-in-amyotrophic-lateral-sclerosis-after-stem-cell-transplantation-into-the-frontal-motor-cortex.pdf?__cf_chl_tk=3qCR37fwN1.loNJjvYhTwguv6pAkUFoI52GR8UG4vEM-1771458258-1.0.1.1-xcBdhixg1w.yUbra9.QpFjD_e6G0FKlscLflUiHcqas\" target=\"_blank\" rel=\"noreferrer noopener\">autologous stem cell transplantation<\/a> can increase both the lifespan and quality of life of those living with ALS.<\/p>\n\n\n\n<p>ALS is the illness that physicist <strong>Stephen Hawking<\/strong> lived with; it affects the motor neurons that control movement, leaving patients unable to speak, walk, eat or breathe on their own.<\/p>\n\n\n\n<p>Globally, <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC6735526\/\" target=\"_blank\" rel=\"noreferrer noopener\">it is estimated that around 350,000 people have ALS<\/a>, and to date, there is no cure. Treatments generally include oral medications, <a href=\"https:\/\/link.springer.com\/article\/10.1007\/s00415-020-09919-9#:~:text=Riluzole%20is%20the%20only%20globally,a%20number%20of%20clinical%20factors.\" target=\"_blank\" rel=\"noreferrer noopener\">such as riluzole<\/a>, which can slow its progression, control symptoms, and improve quality of life.<\/p>\n\n\n\n<p>But according to <a href=\"https:\/\/www.tecsalud.mx\/directorio-medico\/dr-hector-ramon-martinez-rodriguez\" target=\"_blank\" rel=\"noreferrer noopener\">H\u00e9ctor Ram\u00f3n Mart\u00ednez<\/a>, director of the Institute at Neurology and Neurosurgery at <a href=\"\/en\/tag\/tecsalud\/\" target=\"_blank\" rel=\"noreferrer noopener\">TecSalud<\/a>, the available medications are very expensive and their results modest. \u201cThat\u2019s why I\u2019ve always been focused on finding alternatives,\u201d says the expert.<\/p>\n\n\n\n<p>In 2005, he and a multidisciplinary group of doctors and researchers proposed using the patient&#8217;s own stem cells \u2014hence the term autologous\u2014 and transplanting them directly into their brain to regenerate damaged neurons.<\/p>\n\n\n\n<p>To this day, the group has treated 62 patients who have showed a marked improvement in their symptoms and a longer-than-expected survival rate.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">An New Treatment for Amyotrophic Lateral Sclerosis<\/h2>\n\n\n\n<p>The team, led by Mart\u00ednez, designed a protocol in which stem cells are extracted from the patient&#8217;s own blood through a peripheral vein, after stimulating the bone marrow with the drug filgrastim to produce them in excess.<\/p>\n\n\n\n<p>Next, they are grown in the laboratory and an antibody with magnetic particles is added, which allows them to be separated from the rest of the blood contents using a magnet.<\/p>\n\n\n\n<p>Once separated, the cerebrospinal fluid is extracted from the patient and the stem cells are placed in it.<\/p>\n\n\n\n<p>\u201cSince it comes from the patients, we don\u2019t need to use any immune response modulating medication, we eliminate the risk of rejection,\u201d says Mart\u00ednez.<\/p>\n\n\n\n<p>Finally, the stem cells are implanted directly into the motor cortex of the brain through two small holes, called trephines.<\/p>\n\n\n\n<p>Of the first 10 patients treated, none required a tracheostomy or a gastrostomy to breathe or eat, unlike several in the control group who did.<\/p>\n\n\n\n<p><strong>\u201cOf the 10 patients in the control group, five had died within a year of follow-up, and of the experimental group, only two; the survival rate was higher,\u201d<\/strong> says Mart\u00ednez.<\/p>\n\n\n\n<p>At the time, the results were so promising that an international consortium, called <a href=\"https:\/\/neals.org\/about\/neals-member-sites\/\" target=\"_blank\" rel=\"noreferrer noopener\">NEALS<\/a>, focused on ALS research and treatment, requested an audit of their protocol.<\/p>\n\n\n\n<p>\u201cThey spent five days reviewing each file,\u201d Mart\u00ednez recalls. After various evaluations and tests, they approved it and included Tecnol\u00f3gico de Monterrey in the consortium, which includes institutions such as the <a href=\"https:\/\/www.massgeneral.org\/\" target=\"_blank\" rel=\"noreferrer noopener\">Massachusetts General Hospital<\/a> and <a href=\"https:\/\/www.duke.edu\/\" target=\"_blank\" rel=\"noreferrer noopener\">Duke University<\/a>.<\/p>\n\n\n\n<p>The surgery is performed under local anesthesia, so patients can be discharged the next day. Furthermore, no cerebral hemorrhages, infections, or epileptic seizures caused by the procedure have been reported.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Possible Mechanisms Behind the Improvement in Symptoms<\/h2>\n\n\n\n<p>In the treated patients, in addition to the observed improvements in their symptoms and greater survival rate, magnetic resonance imaging showed that the corticospinal tract \u2014the area of \u200b\u200bthe brain that transmits nerve impulses from the brain to the spinal cord to control voluntary movements\u2014 was thicker.<\/p>\n\n\n\n<p>\u201cWhen the patients arrived, this area was very thin; after six months of treatment, it was twice the size, with many fibers,\u201d says Mart\u00ednez.<\/p>\n\n\n\n<p>The researcher believes that the stem cells transplanted into the area promote the recovery of damaged motor neurons, and that this could be one of the physical explanations behind the improvements observed after the procedure.<\/p>\n\n\n\n<p>Although the mechanisms are not entirely clear, stem cells do not generate new motor neurons as such, but rather stimulate the survival and function of those that remain and reduce inflammatory processes associated with degeneration.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">The Future of Regenerative Therapies for Treating Rare Diseases<\/h2>\n\n\n\n<p>Over the years, Mart\u00ednez remembers patients who have made an impact on him and motivated him to continue, such as a woman who came in almost unable to move and who, a few months after treatment, was already standing and bathing herself.<\/p>\n\n\n\n<p><strong>\u201cAnother one arrived in a wheelchair the first time, and the second time, he came in walking,\u201d<\/strong> he says.<\/p>\n\n\n\n<p>Over time, their experimental protocol has evolved into a regenerative therapy platform for treating ALS.<\/p>\n\n\n\n<p>Recently, the team moved to the Zambrano Hellion Hospital where a highly specialized laboratory was developed that will be used for advanced therapies.<\/p>\n\n\n\n<p>\u201cWe are going to use it to perform stem cell transplants not only in ALS, but also in cardiology, hematology and oncology,\u201d says Mart\u00ednez.<\/p>\n\n\n\n<p>In addition, they have other projects, such as working on the early diagnosis of ALS and other rare diseases.<\/p>\n\n\n\n<p>They also plan to continue with the necessary steps so that autologous stem cell transplantation can become a widely used treatment for this disease.<\/p>\n\n\n\n<p>The experience behind the project was presented in December 2025 at the <a href=\"https:\/\/www.ctrms2025.org\/\" target=\"_blank\" rel=\"noreferrer noopener\">18th International Congress of the Cell Transplantation and Regenerative Medicine Society<\/a> in Japan, where it sparked the interest of international groups.<\/p>\n\n\n\n<p>And although the therapy is still in the research phase, and is not a cure, it represents an important effort in Latin America and the world to treat this disease with regenerative medicine.<\/p>\n\n\n\n<p>\u201cWhen a patient is told &#8216;there is nothing that can be done&#8217;, that is where our responsibility as doctors begins: to find something that can be done,\u201d Mart\u00ednez says.<\/p>\n\n\n\n<p><\/p>\n\n\n\n<p><strong><em>Did you find this story interesting?
